Applying FITT Documentation Framework Across the Continuum of Care in Therapeutic Recreation

Session Description

As a crucial part of the APIED process, Recreational Therapists must document their patient’s/client’s progress towards their therapy goals. While the need for documentation in healthcare settings has been well described, there is currently no standardized way to capture what happens in clinical care. In addition, narrative or text-based documentation can vary widely and limit how data can be extracted. Therefore, a new form of documentation was created at our institution to capture the dose of therapy through frequency, intensity, time, and type (FITT) of therapy. In this session, there will be a brief overview of FITT documentation, including the theoretical foundations of the approach. Initial implementation and expansion throughout other service areas will also be described. Finally, future implications of standardized documentation of therapy dose will be discussed.

Learning Outcomes

1. Verbalize the foundation of FITT documentation for therapeutic recreation services at one pediatric hospital 2. Identify the four elements of dose included in FITT documentation (frequency, intensity, time, and type). 3. Explore future implications for FITT documentation at one hospital and within the field of therapeutic recreation

Kristin Greenlee, MA, CTRS, Specialization in Pediatrics, Nationwide Children's Hospital
Biographical Information

Kristin Greenlee, MA, CTRS, completed her undergraduate from Slippery Rock University and graduated from University of Minnesota with a Masters of Arts in Infant and Early Childhood Education. Kristin works as part of a multidisciplinary team to promote leisure and recreation to help individuals meet their functional goals and increase their overall quality of life. Kristin’s primary care roles have included pulmonary, outpatient therapy services, and nephrology. Kristin currently serves as the acute care clinical leader for Therapeutic Recreation, working to continuously advance the field. Kristin is a member of the NCTRC Item Review Committee and the ATRA CPT code subcommittee. She has served on a multitude of conference committees, presented at national conferences, and holds a specialty certification in pediatrics through NCTRC.

Biographical Information

Jamie Sympson is a Certified Therapeutic Recreation Specialist and Clinical Leader within Clinical Therapies Behavioral Health at Nationwide Children’s Hospital (NCH), with nearly a decade of experience supporting children, adolescents, and caregivers across complex behavioral health and pediatric pain care settings. A graduate of the University of Toledo, Jamie holds a bachelor’s degree in Recreational Therapy and earned a Behavioral Health specialization designation through the National Council for Therapeutic Recreation Certification (NCTRC) in 2021. Since joining NCH in 2017, Jamie’s work has focused on delivering trauma‑informed, patient‑centered and behavioral health care across acute and intensive treatment environments. In addition to providing direct Therapeutic Recreation patient care, Jamie provides clinical leadership and direct supervision to Speech‑Language Pathology and Therapeutic Recreation team members across inpatient pediatric behavioral health units, the Child and Adolescent Partial Hospitalization Program, and the Intensive Pain Rehabilitation and Education Program. Jamie currently serves as a Clinical Lead and HUB member for the Pediatric Chronic Pain Foundations ECHO at NCH and has been an active member of the ECHO team for the past four years. Jamie is also an active member of the Ohio Recreation Therapy Association and the American Therapeutic Recreation Association
